Our Admissions & Intake Process

Coming to terms with the fact that you or someone in your life needs treatment for a behavioral or mental health concern can be extremely eye-opening. And it can also be extremely overwhelming as you begin navigating the possible options for care that may be of most benefit to yourself or your loved one.

At Crestwyn Behavioral Health, we understand that the prospect of seeking treatment for a mental health, substance abuse, or dual diagnosis concern can be quite intimidating, especially if the prospective patient has not received behavioral health treatment in the past. For this reason, we have designed our admissions process to be a simple, streamlined, and worry-free experience for all who seek out our services.

The First Step

To meet the needs of potential patients and referral sources, Crestwyn makes assessments for admission accessible 24 hours a day, 7 days a week. Those seeking support can speak with a member of our staff to inquire about treatment, schedule an assessment on a day and time that is convenient, or begin the process of admissions over the phone by contacting our Assessment and Counseling Referral Office.

During the admissions assessment, the following will occur:

  • Demographic information, clinical history, and insurance and payment information will be gathered by an assessment counselor.
  • An assessment of presenting concerns will be completed in order to clarify the types and levels of care that may be needed by the prospective patient.
  • If the potential patient is not on site, an in-person assessment will be scheduled. If the individual is already at Crestwyn, his or her needs will be assessed at that time.

What to Expect When You Contact Us

In order to receive treatment at Crestwyn Behavioral Health, an individual must meet the following criteria:

  • For admission to our adolescent services, the individual must be between the ages of 13 and 17. We may be able to admit individuals as young as 12 years old if they are developmentally, physically, and clinically appropriate.
  • For admission to our adult treatment, a person must be at least 18 years old.

Additionally, an appropriate patient for Crestwyn must present with one or more of the following concerns:

  • Demonstrates that one poses a threat to oneself or others and requires professional observation, assessment, and intervention
  • Engages in self-destructive behaviors that places the individual’s or another’s life in peril
  • Conveys significant verbal threats regarding one’s safety or the safety of others (within three days of admission)
  • Displays significant behaviors that are assaultive in nature that threaten the safety of the individual or others (within three days of admission)
  • Communicates homicidal ideation (within three days of admission)
  • Shows severe disturbances in mental functioning
  • Displays cognitive impairment, such as disorientation or loss of memory, due to the presence of a mental health disorder that places the personal safety of the person and others at risk
  • Suffers from hallucinations, delusions, panic, extreme anxiety, depression, or other such concerns that result in unusual behavior and an inability to function daily or reduce risk of harm to oneself or others
  • Demonstrates a significant decrease in functioning across several areas of life that necessitates acute inpatient treatment
  • Experiences adverse reactions due to a psychotropic medication in which serious side effects may occur
  • Shows a significant amount of weight lost or gained that is not due to a medical condition
  • Presents with significant psychiatric and/or behavioral concerns while also suffering from early onset Alzheimer’s disease
  • Requiring a higher level of care than outpatient treatment can provide due to severe symptoms, a complex diagnosis, or aggressive behaviors that cannot be treated in a lower level of care or through another type of service

If you, a person you care about, or an individual that you are treating meets one or more of the above criteria, then an admission to Crestwyn Behavioral Health may be appropriate.

Following the completion of the admissions assessment, an assessment counselor will review the information gathered and consult with one of our psychiatrists for final approval for admission. Once done, the assessment counselor can then pre-authorize inpatient care with the patient’s insurance provider.
